2018 membership form???

I'd be grateful if someone could direct me to the latest membership form, please. I can find the 2017 form on't website, but not the current one...

I have a copy drop me a pm and I'll email it you but you don't need to fill it in if you were a member last year here's what the email says

Membership fees are same as last year £10 for an adult member and £5 for under 16. We will then add you to the 2018 membership list, no need to complete a membership form. The gate code will be changed next week and all 2018 members will be email the new gate code.
