Specifically on the Yoko, perhaps (surely so) - but anyone that's had a car that uses this sort of chassis design (losi, kyosho, ae, etc) has probably had it happen from time to time. It's also part of the reason for this design, that the user only has to replace the t-plate instead of the entire chassis.
As far as why it happens - it's usually as a result of user error, bad jump landing or major crash.
